  • Payment

    We are an out-of-network provider. Payment at time of service is expected. We will provide you with a detailed superbill in which you can submit to your insurance for reimbursement.

  • Medicare

    We are a Medicare B out-patient therapy provider and will be happy to submit your claim. Please be advised, due to Anchored Shoulder to Hand Therapy not filing secondary insurances, you may be responsible to pay 20% of the submitted claim. A bill will be issued for said 20% once Medicare payment is received. Prompt payment is expected. We do not participate in Medicare Advantage plans.
